These terms apply to appointments with Dr. Kimberly Weinreich, DACM, L.Ac (“Dr. Kim”) at Sollua (3660 Clairemont Dr, Suite 4, San Diego, CA 92117) and Root & Soul (9131 Fletcher Pkwy, Unit 108, La Mesa, CA 91942).

 

Booking and payment

Appointments are booked online. A valid credit card is required to book and is kept on file to hold your appointment. Payment is due at the time of service. Listed prices are starting rates and may vary by treatment.

 

Herbal formulas are charged in addition to the consultation

 

Cancellation policy

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a gap in the practitioner’s day that could have been filled by another patient. We require 24 hours’ notice for any cancellation or change to your appointment. Patients who give less than 24 hours’ notice, or who miss their appointment, will be charged a 100% cancellation fee to the card on file.

 

Consent to treatment

Before treatment, you will complete patient paperwork, including consent forms specific to the services you receive (such as acupuncture and associated therapies, facial acupuncture, microneedling, herbal medicine, and other treatments). Please read these carefully, tell Dr. Kim about all medical conditions, medications, and allergies, and ask any questions before care begins. You may withdraw consent and stop treatment at any time.

 

Medical disclaimer

Acupuncture and Chinese medicine are complementary care and are not a substitute for diagnosis or treatment by your physician. Results vary and no outcome is guaranteed. In an emergency, call 911.

 

Privacy

Your personal and medical information is kept confidential. With your permission, Dr. Kim may communicate with your family doctor or referring doctor when needed for your care. Information is otherwise disclosed to third parties only with your permission.

 

Insurance

No insurance currently accepted however a Superbill can be provided for reimbursement from insurance companies that accept out of network acupuncture. HSA/FSA eligibility varies by plan.

Herbal products

Herbs are non-refundable. Take herbs exactly as directed, store them safely away from children and pets, and contact Dr. Kim if you have any reaction.
